Stained Edition - Safety & Liability Waiver
1. ASSUMPTION OF RISK
I understand that stained glass workshops involve working with sharp glass, soldering irons, chemicals (such as flux), lead and other tools that may cause injury. Lead work is not recommended for those pregnant or breastfeeding. I voluntarily choose to participate, understanding and accepting these risks.
2. AGE REQUIREMENT
I confirm that I am 16 years of age or older. Participants under the age of 18 must have a participating (registered and paid) guardian co-sign this waiver and be accompanied at all times.
3. SAFETY GUIDELINES
I agree to follow all safety instructions provided by Stained Edition instructors. I will wear closed-toe shoes and use protective gear (glasses, gloves, aprons) as directed. I understand that I may be asked to stop participation if I do not comply with safety procedures.
4. FOOD & DRINK POLICY
Food and drink may be consumed at your own risk. Drinks must be in sealable containers. Any food or drink stations must be located at least 6 feet away from glasswork areas.
5. ALCOHOL CONSUMPTION POLICY
All alcohol consumption is at the participant’s own risk. Alcohol must be consumed responsibly and must not interfere with safe handling of tools or materials. Any visibly intoxicated participant may be asked to stop working for safety reasons without refund. Participants must be of legal drinking age in the event location. Stained Edition is not responsible for any incidents, injuries, or legal issues arising from alcohol consumption.
6. TOOL & MATERIALS POLICY
All tools and equipment remain the property of Stained Edition. Participants will take home their finished stained glass project and may take home any leftover glass from their individual kits. It is the participant’s responsibility to transport any glass or artwork safely.
7. HEALTH & LIABILITY RELEASE
I affirm that I do not have any condition that would increase risk of injury during this workshop or, if I do, I accept full responsibility for informing the instructor and managing my participation. I hereby release Stained Edition, its owners, employees, and affiliates from any and all liability for injuries, losses, or damages that may result from my participation.
8. PHOTO & MEDIA CONSENT
I give permission for photographs or videos taken of me or my artwork during the workshop to be used for Stained Edition marketing purposes.
9. EMERGENCY CONTACT (optional)
Providing emergency contact information is optional but encouraged. This is especially important for participants with health concerns or unique support needs.
ADDITIONAL ACKNOWLEDGMENTS
10. BROKEN GLASS & RESIDUAL HAZARD DISCLAIMER
I understand that even after thorough cleanup, small glass fragments or residue may remain. I will take appropriate precautions during and after the workshop, including checking my clothing, belongings, and shoes for glass before leaving. Stained Edition is not responsible for any injury or damage caused by residual glass after the workshop.
11. HALTED PARTICIPATION & EVENT TIMING POLICY
I understand that if I am asked to stop participation due to safety violations, intoxication, or disruptive behavior, I will not receive a refund. In timed workshops, self-caused delays or interruptions may result in an incomplete project, and Stained Edition is not responsible for unfinished work.
12. REFUND POLICY
Stained Edition does not give refunds or credit for classes unless we cancel it. It is your responsibility to remember your class date and time and to arrive prepared.
13. RESPONSIBILITY FOR PERSONAL PROPERTY
I acknowledge that I am solely responsible for my personal belongings, including phones, bags, and other valuables. Stained Edition is not liable for loss, theft, or damage to personal property during the workshop.
14. GUESTS
Participants may not bring guests. All persons permitted into the venue or workshop area must be a paid participant and observe all rules outlined in the waiver.
15. MEDICAL EMERGENCY CONSENT
In the event of a medical emergency, I authorize Stained Edition staff to seek emergency medical assistance on my behalf and agree to be responsible for any resulting medical expenses.
